CO.  Isn't this more of an organizational problem?  The problem is we don't always know when we stink. A Cornell University study showed that people who bombed tests of logic, grammar and humor "grossly overestimated" how well they did on the tests.  "Not only do they reach erroneous conclusions," said the authors, "but their incompetence robs them of the metacognitive ability to realize it."  It is even worse when the status of the organization in society prevents oppositional thinking.  For example: the US defense establishment and the news media.  Can they truly be transformed without suffering a resounding loss of prestige?

Frontline on the terrorist Web.  Simple techniques (it's funny to contemplate how difficult these were to figure out for people that don't live a large part of their life online) for powerful results.  The state's loss of control over communications is a seismic shift.
